Through the Grant for the Benefit of Homeless Individuals, Bacon Street Youth and Family Services is excited to announce the launch of the Journey Home Project. This project will provide Counseling and Case Management services, free of charge, to youth ages 10-26 and their families who are homeless or precariously housed.

Qualifications:

  • Youth and their family members ages 10-26
  • Seeking Clinical or Case Management Services
  • Struggling with Homelessness or Precariously Housed
  • Struggling with a Substance Use Disorder or Co-Occurring Substance Use and Mental Health Disorder (This can be the youth and/or their family member!)

What We Can Provide:

  • Mental Health Counseling Services
  • Substance Use Counseling Services
  • Case Management/Referral Services
  • SOAR SSI/SSDI Outreach, Access, and Recovery Services
  • Group Counseling Services
  • Bus passes for transportation
  • Childcare passes
  • Our Hampton site will feature a teen “drop-in” center where we will serve two meals per day, offer laundry services, and shower services.
